Let Us Know if You Have Not Received Your 1099-R

With April 15 fast approaching, it is imperative that you get in touch with us if you have not received your 1099-R form. These forms were mailed out in January.
In the event that your 1099-R has an incorrect address, we ask that you download, complete and sign a Change of Address form (available here) and attach a note requesting a new 1099-R. You can mail or fax this information to our office.
In general, we request all our participants to send us signed Change of Address forms to ensure that participants receive important Fund information on a timely basis. |
