Form 1099 R's will be mailed by January 31, 2007. These forms serve the same function as a W2: they show how much money you received from us during the course of the 2006 tax year. These forms are essential for completing your taxes.

If you have not received your 1099 by February 7, 2007 please give us a call for a duplicate copy. In the event that your address has changed, please send us a signed Change-of-Address form which can be found here.
